1. Club RoAnime and its hosting location are not responsible for any lost or stolen items left on the premises.
2. All minors (those under the age of 16) MUST be accompanied by a parent (or other family member) or a legal guardian (Who must be over 21 years of age) and who will be solely responsible for guidance of viewing any show at the club. Items that can be shown can include adult content up to and including wanton and gratuitous violence, nudity, adult scenes and situations, and harsh language. Club RoAnime will have any parental guidance suggestions noted on the home web page at www.clubroanime.com, however the allowance of viewing is the sole responsibility of a minor’s accompanying adults.
3. Those minors ages 16 – 18 are also subject to Rule #2 as stated above. But this can be waived by having the minor’s parent or legal guardian signing a separate form, and having it notarized that the minor in question is to be treated as a legal adult, and that the parent or guardian will hold no liability to the club as a result of said minor’s actions.
4. Club RoAnime reserves the right to deny or terminate membership for anyone at anytime, with or without reason.
5. The executive staff of Club RoAnime also reserves the right to modify these rules as needed, and will serve notice of said changes via verbal announcements at the beginning of club meetings, on the club mailing list, and upon the club web page

I _____________ the parent / legal guardian of ________________ (hence forth referred to as minor) do herby declare that for the purpose of the Club RoAnime meetings and events that my minor is to be treated as an adult. And that I hereby state that said minor has demonstrated rational judgment and good moral character and is fully capable of behaving as an adult in normal society. I also will not hold Club RoAnime itself, or its executive members, or the hosting location liable for actions words or deeds taken by said minor while attending a Club RoAnime function. I also understand that said minor is still bound by the terms and conditions of the full membership form.

I am also aware that Club RoAnime may show items that can include adult content up to and including wanton and gratuitous violence, nudity, adult scenes and situations, and harsh language. I also understand Club RoAnime will have any parental guidance suggestions noted on the home web page at www.clubroanime.com, however the allowance of viewing is the sole responsibility of said minor.

I understand that Club RoAnime reserves the right to deny or terminate given membership for anyone at anytime, with or without reason.

This contract as stated above I do fully understand and have discussed with said minor and we both do agree to these terms and conditions.
